I had bumper to bumper car accident on 2/16/16 and went to Ivy Urgent Care the next day on 2/17/16 because of some pain I had some tests (X-Rays) and visual description from doctor. IVY Urgent Care first, recommend me to Dr. John Insalata at 25 Manor Rd, Staten Island, NY 10310 (I never have any treatment from this facility since they have referred me to go another facility), from there they gave me the address to go to The Spine & Pain Institute of New York in Staten Island, so I set up an appointment and went on 2/27/16. After finished with The Spine and Pain Institute, They referred me for physical therapy to NY Chiropractic & Physical Therapy (NYCPT) located at 1163 Forest Avenue Staten Island, New York 10310, and started to undergoing treatment from 3/14/16 to 6/14/16. When I started to go on first day, 3/14/16, they have explained what procedure they will start on me, and started setting up appointments for Chiro/PT treatments (That include chiropractic treatment, use of Jeanne Rub manual massage tool and electronic mini massage, they also used a heater pad when started the treatment),. In the beginning everything went well, but then, one day, about two-three weeks starting from Chiro/PT then one day, I have a call asking me for an injection to see if I had a "nerve damage” and I say NO, then when I went there they were asking me if I’m still interested to have this “injection to check if I have any nerve damage” again I said NO.
When I continuing going to this facility, everything seemed like really nice people when they explained me the procedures of PT and Chiro ( they also explain me that Chiro treatment will sound weird with the cracking noise) so I let them do it and I believe I did it a few times (in a private room first time and second time), then I was taking this treatment to another larger room where there is PT beds with curtains on the side, until I said no more of this Chiro treatment (could not deal with the “cracking noise”), then they had starting continuing with the Jeanne Rub variable speed massager – Core Products International treatments which consist using manual massager back and forth for the back on each sides (not directly to the spine), but noticed every time I have this treatment next day I woke up with some pain, there was and I do remember one particular day, (around 4/25/16 @ 6:00pm) a PT Staff named “Mike” did the massage on that day, he used the manual machine back and forth on my back and then he used the manual machine directly into my spine. That day I felt weird, (somehow I felt something is wrong and took pictures of the bed and machine)( I have day and time of this incident)_ and then one of the Staff name Eldin Mehanovic, said to me “don’t worry everything will be fine we take care of you”.
So I get up with some weird pain, took my coat and left. Next day I called them to stop using the manual massage with the machine for the back, because I felt so much pain. So they (NY Chiro and PT) set up me appointments with Dr. Rushit Katharotia for only neck physical therapy and some hand back massages “only” with also a Pulse Massager Mini pads that stick in your back and including heating pads, and I continued taking only this type of treatments approximately one to two months until I stopped.
I was unable to see my primary Dr. Vladimir M. Kozhinskiy, to follow up because that period of time I did not have any medical insurance and he does not accept Geico No Fault coverages. I only relied into my Geico No Fault Insurance coverage to see a no fault doctor and treatments. Because of continuing dull pain, So I called Geico for an additional tests and went back to Ivy Urgent Care for the second time on 9/14/16, after they saw me on that day, they have referred me to go to Regional Radiology at 360 Bard Ave, Staten Island, NY 10310 for an MRI Cervical Spine test, for ultrasound abdomen complete test and Non Obstetrical Limited Pelvic test, So I have my MRI test done on 9/19/16 at Regional Radiology Staten Island NY, within 24 hours I got a called and was surprised when I was told I had a herniated disc (protrusion disc C5-6) and two herniated back disc This injury I have never previously had before, not even a previous accident since I actually never have an accident prior to 2/16/16, and does not reflect on my first time X rays test on 2/17/16. Only after I went to the IVY Urgent Care again on 9/14/16 and to Regional Radiology on 9/19/16, I have learned that I have a new condition. The other tests I have done was on 9/23/16 “for ultrasound and pelvic”. I recently finally have medical insurance, and the available appointment is by Dec 29th, 2016 to follow up with my condition and pain management and let the doctor know about the accident and personal injury.
After going to the appointment and see my primary doctor: Dr. Vladimir M. Kozhinskiy, 242 Mason Ave Ste 2, Staten Island, NY 10305 on 12/28/16, my doctor referred me to see a Neurosurgery, Pain Management
